GENERAL MEMBERSHIP QUESTIONS

Can I Add Extra People To My Membership?

Wellphoria welcomes additional family members. New Program members must open their own account and are welcome to add up to four (4) additional members of their immediate family for an additional charge.  Each additional member will receive his or her own Card.  Members may add additional family members by sending an e-mail to hereforyou@wellphoria.com.    

How Do I Remove Extra People From My Membership?

Extra people can be removed within 30 days of their addition to the membership.

Can I Become a Member if I am Under 18 Years Old?

At this time, program memberships are not available to minors under 18 years of age.

I Forgot to Show My Card – Can I Still Receive My Discount?

You must show your card to the provider to receive your discount.

LOST MEMBERSHIP CARD

How Do I Get A New Card Sent To Me?

If your Card is lost or stolen, Wellphoria charges a $5.00 replacement fee and $2.00 for shipping and handling, $7.00 total. You can report lost cards by contacting Wellphoria Customer Service, at 1-800-well-4-ya, or by email at hereforyou@wellphoria.com

BILLING

Am I set up for automatic membership renewal?

Charges will automatically renew yearly, for the dollar amount, and for the billing periods that are specified in the registration process. If the credit card authorization fails, we will request updated credit card information. However, if you do not provide updated credit card information that successfully authorizes within 72 hours after notice, we may terminate your Program membership.

Can I Pay By Check?

At this time, Wellphoria accepts only credit cards as payment.

I Have a General Billing Question – Who Can I Call?

Why does my card have an expiration date on it?

Every Wellphoria card has an expiration date stamped on it. Our cards expire one year after the date of issue, and Wellphoria will automatically send you a new card one month prior to expiration of your old card.

How Can I Add My Favorite Provider to Your Network?

How Do I Search for Providers of a Certain Service, or Within a Certain Geographic Area?

My Provider Didn’t Recognize My Card – What Do I Do?

Rest assured that every provider listed online in the Wellphoria Provider Directory is current. If your provider doesn’t recognize your Wellphoria card and offer you the published discount, please call 1-888-Well-4ya or email hereforyou@wellphoria.com to report the problem to a representative.
         
My Provider Declined My Card After I Received Service – What Do I Do?

Have your provider contact  the following number, as listed on the back of your card, to verifiy eligibilty. 1-800-274-7526.

CANCELING MY SUBSCRIPTION

How Do I Cancel My Subscription?

New members who cancel within 30 days of the Effective Date will receive a full refund of all fees paid to participate in this plan less the non-refundable enrollment fee.  Members seeking such a refund must request cancellation in writing and mail it and their membership card request to Wellphoria Inc. 470 Atlantic Ave., 4th Floor, Boston, MA  02210 within 30 days of the Effective Date.
